[FIXED] Clustering: clients possibly not removed due to removed HB timer

In some situations, it could be possible for a node becoming leader
to not properly register (actually unregister) the client's timer
supposed to check for client health (response to server HBs). This
could lead to clients and their subs not being removed from the
server.

Fixes #791
