Improve error & panic metrics (#169)

* Add ndt5 metrics for connections, results, errors, and panics.
* Remove global ndt_test_total and ndt_test_errors_total
* Improve meta error handling and metrics
* Add error counts for login and status check
* Add protocol and direction labels throughout
* Update README with ndt5 monitoring metrics
