Only rewrite slice references for groups that were actually moved

_rename_slice_groups built the HDF5Reference rewrites in the planning
loop and applied them unconditionally, while the h5py move was guarded
by 'source present in file'. If a slice's referenced group was missing
from the auxiliary file, the move was skipped but the reference was
still repointed at a destination group that was never created, leaving
it silently dangling.

Tie each reference rewrite to a successful move: plan per current group,
skip (and warn) when the source is absent, and apply only the updates
for groups whose data was moved.

Add test_rename_leaves_missing_group_reference_untouched (fails on the
unconditional-rewrite code, which repointed at the non-existent group).
