chore(ci): add a coveralls configuration file (#615)

The `coveralls` devDependency ships a CLI that could upload coverage, but it
had no configuration to read. `coveralls@3` only honours `repo_token` and
`service_name` from `.coveralls.yml`, and it has no GitHub Actions detection,
so `service_name` has to be declared for the CLI path to report correctly.

The CI upload is unchanged: `.github/workflows/coveralls.yml` keeps using
`coverallsapp/github-action` with `GITHUB_TOKEN`. The repo token is a secret
and stays in `COVERALLS_REPO_TOKEN`, which is why the file documents it rather
than storing it.
