Add release notes to the Sphinx build (#42)

The releasenotes/ directory, reno's config, and `reno.sphinxext` in conf.py
were all already in place, as was `reno lint` in the lint env and the docs
workflow step that renames the unreleased section after the current branch.
The only missing piece was a page invoking the directive, so the notes were
being collected but never rendered.

Adds docs/release-notes.rst and lists it last in the index toctree, matching
the convention used across the other Qiskit addons.
