Fix the smaller findings of the review of boltlang-hir-ty

- `impl_assoc_ty` is a query, since solving any projection through an
  impl asks for it, and it now substitutes the class's defaults.
- `unmet_assoc_equalities` and `unmet_assoc_bounds` assume an impl's
  bounds once, in a solver that each check clones, rather than once per
  equality or bound.
- The doc comments of `assume` and `unmet_assoc_bounds` were on the
  functions before them, and a comment in `unmet_assoc_equalities` still
  said that a type an impl doesn't give is the error type.
- An overload constraint keeps each candidate with its type in one
  vector, rather than two vectors kept in step.
