fix: self-review fixes for the 0.8.0 features

- from_strands reports the last call's token usage; accumulated_usage adds up
  every call on the agent while the steps are only those in agent.messages
- assert_tool_calls_allowed checks keyword values (required, enum, pattern,
  properties, bounds, lengths) and raises ValueError for malformed ones; a
  huge integer no longer overflows the number check
- an empty tool call id is None, as in the adapters
- tool call sequence failures leave call ids out (expected calls have none);
  ToolPolicyError keeps them
- --cassette-report lists a file whose metadata is not a mapping as
  unreadable, and says to run without pytest-xdist workers when it finds
  nothing under xdist; the README states what its status column covers
