Ignore surplus row values in columns and facetcolumns (#771)

`columns()` and `facetcolumns()` can append surplus row cells to a real
column when `missing` matches that field's name. For example,
`columns([['NA', 'value'], ['kept', 10, 'extra']], missing='NA')`
currently returns `['kept', 'extra']` for the `NA` column. An unhashable
filler such as `[]` or `{}` instead raises `TypeError` on long rows.

Limit row materialization to the fields in the header, padding only
missing values. The regression tests cover both functions, multiple
facets, long and short rows, a colliding filler, unhashable fillers, and
the default filler. Document the behavior in both docstrings and the
changelog.
