fix(api): handle nil pointer safely in RuleTypeReleasePhase.EnsureDefault (#6967)

EnsureDefault dereferenced the receiver pointer *s when s == nil, causing a runtime SIGSEGV panic on nil pointers. This also caused InitializedStringValue() to panic when invoked on an uninitialized phase.

Fix this by allocating and returning a default GA phase when s == nil, matching the safe behavior of Severity.EnsureDefault(). Also add unit tests covering nil, unspecified, and explicit phases for both EnsureDefault and InitializedStringValue.
