* fix: bind fresh runs to a generated session id

Transcript selection took any fresh file in the cwd project dir that contained
the prompt, so two overlapping fya runs in one directory with the same prompt
could pick the same transcript. One run returned the other's answer with exit
0, then killed its own Claude mid-turn.

A run without session flags now starts Claude with a generated UUIDv4 as
--session-id and reads <id>.jsonl directly. A caller's own --session-id is
used the same way, and an empty one is rejected. Runs with --resume,
--continue, --fork-session, --from-pr or --worktree let Claude choose or move
the session, so they keep the prompt search.

* fix: pin debug flags to session binding, align session-id error

-d and --debug must leave the generated session id in place, but nothing
tested it: the short-flag check was the negation "anything but -d", so
widening it would have sent every debug run back to the prompt search
without a failing test. The check now names -r and -w, and test rows cover
-d, --debug and -v.

The empty --session-id error now reads "flag --session-id ..." like the
other parse errors, and the ErrNoTranscript godoc and the package overview
name the session-id lookup.
