Add the statuses Laravel route middleware answers with, and Passport scopes (#20)

A Laravel route now gets a response for each status its middleware can
answer with before the handler runs. Its security requirements also
carry Passport's scopes.

- `verified`, `signed`, Sanctum's abilities, Passport's scopes and
spatie/laravel-permission's role and permission middleware add a `403`,
next to `can`. `password.confirm` adds a `423`, and `throttle` a `429`.
- Laravel's own middleware counts by alias or class, subclasses
included. The packages' middleware counts by alias or class name,
without the packages being installed.
- The map of middleware to status code is the adapter's `middleware`
option. A project extends its defaults for middleware of its own. It
replaces the `authorization` option, which has not been released.
- `scopes:read,write` puts both scopes on each security requirement.
`scope:read,write` gives one requirement per scope.
- Responses come out in status code order, described with the standard
status texts.
- A response the attributes declare for the same status code wins.
