Keep the 404 for a bound model parameter that has a default (#19)

A route parameter bound to a model now gets its `404` even when the
handler gives it a default. Laravel only skips the binding when an
optional placeholder is missing from the URL. Where the placeholder has
a value and no row matches, it answers `404`, default or not.

- On a route with an optional placeholder, the path that has the
placeholder gets the `404`, and the path without it does not.
- A required placeholder bound to a parameter with a default now gets
the `404`. Before, it got none.
