Move the documentation into docs/ and document the adapters' options (#17)

The README now holds the introduction, requirements, installation, a
short usage example and links. The rest of the documentation moves into
`docs/`, one page per topic, and each adapter option is documented with
its default and an example.

- The usage example sets up the builder first, as for swagger-php on its
own, with registering the introspector as the one added step.
- `docs/adapters.md` lists every adapter option, says which adapters
take it and links to where it is explained. The handler and constraint
options each get an example of extending them.
- Laravel's model binding, form requests and authentication have their
own page. "What wins" keeps only the rules for how routes and attributes
fold together.
- The inventory page now starts with what the inventory is: the
introspector's own output, recorded during the build.
- Routes, L5-Swagger and response verification each have a page, with
the content the README had.
