Let the L5-Swagger factory map guards to schemes and take a logger (#16)

The L5-Swagger builder factory now uses the application's default guard
for a bare `auth` middleware. It can also map guards to security schemes
and give a logger, each through a method a subclass overrides.

- A bare `auth` uses `auth.defaults.guard` from the application's
config, `web` where it sets none.
- `schemes()` maps guards to security schemes, for example something
like `['sanctum' => 'bearerAuth']`. A route whose guards all map gets
their security requirements. It is empty by default.
- `logger()` is the logger the builder and the adapter report to, so a
form request whose rules cannot be read shows up in the log. It returns
none by default.
- Without an override, the generated document is the same as before.
