Publish Coveralls coverage and expand CLI integration tests (#3)

Collect rule coverage across unit tests and CLI subprocesses with c8. Publish LCOV from the Node 24 test job, add a coverage badge, and exercise report-only fixes, relative project vocabulary, suppression, and missing dictionary failures through the CLI.
