fix(chat): load shiki on demand and fix Markdown links and code blocks (#2494)

- renderer: shiki loads with a dynamic import only when highlighting
  is on, so a noHighlighter page no longer loads about 128 KB (40 KB
  gzipped). A local walkTokens replaces marked-shiki
- highlighter: only the light and dark themes in use load, and one
  theme for both modes goes as theme, so shiki tokenizes once.
  Renderers with the same languages and themes share one highlighter.
  An unknown theme skips the highlighter, a partial theme object takes
  the default for the other mode, and 'none' next to another theme
  uses a plain theme, because light-dark() rejects inherit
- streaming: an LRU cache keeps the finished code blocks of a message.
  An open block up to 2 KB highlights without the cache, a larger one
  shows as plain code until its closing fence arrives.
  createMarkdownRenderer parses a message again only when its text
  changes
- code blocks: a block that shiki cannot highlight shows as escaped
  plain code with a language class, not as HTML
- links: the link renderer wraps the one from marked, so link text
  keeps its formatting and href and title are escaped. The default
  sanitizer adds a DOMPurify hook for the call only. The hook opens
  a, area and SVG links in a new tab with rel="noopener noreferrer",
  and the global DOMPurify config of the app still applies
- colorReplacements: a custom map applies to every theme. The GitHub
  defaults stay on the GitHub themes
- deps: drop the marked-shiki peer dependency
- tests: first spec for the renderer, 30 tests
- docs: changelog
