Sheet editing: read a hysprint sheet's layout from the file

The KIT lab edits their own Excel instead of narrating the whole process,
and each upload may have different steps, order and column counts. So the
layout comes from the file itself: row 1 step groups (forward-filled like
the batch parser, merging not required), row 2 headers, rows 3+ samples
labelled by the Sample column.

Tests run on a synthetic KIT-shaped sheet built in code; the real KIT
sheet is private and only tested locally when docs/ has it.
