storage_access: Remove duplicate SM records (#7335)

This is a continuation of
https://github.com/xapi-project/xen-api/pull/6873

If multiple hosts have installed a new SM plugin since the last
toolstack restart and are restarting simultaneously, there's a race
window between checking for registered plugins and registering a new
one, which can lead to duplicate SM plugins being registered.

I was not able to reproduce this myself, but we've seen this case in
support:

Storage_access.on_xapi_start: found duplicate SM plugins for the same
type: type:linstor...

Clean up these duplicate entries instead of just logging them.

Sort the records by reference to get deterministic ordering from
multiple hosts. Remove all but the first such duplicate record.
