feat: rules for which log entries page Monitor, by plugin and level (#63)

Error alerts had one threshold, one level-to-colour map and one cooldown for
every plugin. They are now a table of rules on Monitor Settings, each naming:

- a plugin (log channel), or * for any plugin
- the least severe level it covers, from emergency down to debug
- the alert it rings at: red, yellow, blue, or off to silence it
- a timeout in minutes that holds repeats of the same error back

An entry takes its own plugin's rule when one covers its level and the *
rules otherwise; of those, the most severe level it reaches wins. Excluded
channels become rules set to off. The hourly cap stays global.

Rows saved before the rules are read as the rules that behave the same, so
an upgrade changes nothing until the form is saved. The sender now takes the
alert level from the matched rule, and the throttle the rule's timeout.
