* fix(realtime): stale subscribe no longer resets the rejoined channel

* fix(realtime): keep a stale subscribe from losing its close error or joining

Read and clear the server-close marker before the stale-task check so a
subscribe that was replaced before its catch ran still throws
channelClosedByServer instead of a bare CancellationError, and the marker
no longer keeps the finished task referenced.

Check for cancellation after ensureSocketConnected() returns so a subscribe
attempt that was cancelled while the socket was connecting cannot overwrite
joinRef and send a second phx_join for the replacement subscribe.
