join an in-flight refresh of an expired entry

A refresh of an expired or collected entry replaced any registration,
so while another refresh's loader was still running, each caller
replaced the last and loaded again: 8 refreshes made 8 loads and kept
only the last value. Such a refresh now joins the placeholder and
takes the linearization point of the refresh it joined, so a reload
whose entry expired during it is discarded at commit as it is when the
entry expires after the join. This also rejects a loader refreshing its
own key without a separate owner check.
