molly: drop a generator from a mix only when it is exhausted

mix_gen() removed a generator from the mixture when it yielded a
nil value, even if the generator was still alive: in the iterator
protocol a generator is exhausted only when its first return value
is nil. A live generator that yields (state, nil) was therefore
discarded prematurely and its remaining values were lost.

The generator is now removed only on exhaustion. A nil value
returned by a live generator advances its state and makes mix_gen
retry the selection, so no nil values leak to the consumer.
