Name the Houston override for where it applies, and the fields for their product (#2384)

Review fixes:

- internal.apc.localhost.houston_url becomes internal.apc.houston_url:
  it is the Houston URL for a context whose domain is houston as well as
  localhost, so "localhost" in its name misled.
- The Go fields say which product they are for: APCAPIPort,
  APCWSProtocol, AstroLocalCoreURL, APCHoustonURL, rather than Cloud* and
  Local*, which read as Astro's.
- Tests refuse the other renamed 1.x names, cloud.api.ws_protocol and
  local.houston, naming their new ones.
- The upgrade doc lists which cloud.api.* settings moved, and says
  cloud.api.token was removed rather than moved.
