* fix(prompts): route low-confidence singletons to verify instead of dropping them

in a run with no degraded source, synthesis dropped an uncorroborated single-source finding below confidence 80 unless it was critical or major. The finder schema scores confidence by how far the failure was traced, so the cut removed findings whose missing step is exactly what verify checks, before verify ever saw them. Discussion 48 measured one: a minor at 50, dropped, refiled three rounds later and confirmed.

rule 5 now drops nothing, which also covers the degraded-run clause it carried. The schema descriptions require every input in exactly one output, while the parser still tolerates an unclaimed one and EventDropped reports it. The triage profile keeps --no-synthesis for the confidence boost alone, and the docs, site and both skill trees say so.

* fix(prompts): preserve synthesis prompt overrides

the embedded schema required every input to appear in exactly one output, contradicting synthesis prompts that still drop findings, including files kept from earlier revmux init runs. Allow at most one output per input and leave the no-drop policy in the shipped prompt.
