fix: write flag events when an environment is removed from a project

Removing an environment from a project turned its flags off there
without a flag-level event, so the revision SDKs poll didn't move: they
kept getting a 304 on their old ETag and served those flags as on until
some other change came along. Each flag that was on now gets a
`feature-environment-disabled` event, the same one a bulk disable
writes, so integrations listening for it post once per flag. The startup
environment remap goes through the same path and gets them too.
