Run CI before releasing and create GitHub releases

A pushed `v*` tag now runs the whole CI (ci.yml as a reusable workflow) on the
tagged commit, and the PyPI upload waits for it and uploads the sdist and wheel
the CI built and tested instead of building them again. Afterwards a GitHub
release is created with the packages attached and the version's section of
CHANGELOG.rst as notes, extracted by tools/release_notes.py. The notes are
extracted before the upload, so a final release whose changelog heading still
says "(unreleased)" stops before anything is published. Pre-releases such as
v4.0b1 use the upcoming version's section and are marked as pre-releases.

The CI's concurrency group no longer uses `github.workflow`, which is the
caller's name in a called workflow and would deadlock the release run. Also
drop the v4.0 branch from the CI triggers, it is merged into main.
