auto-review: share the steer-interruption note with plan reviews

Plan reviews spawn and wait on child reviewer threads too, so move the
note into STEER_INTERRUPTION_NOTE and add it to every plan-review opener.
Tell the agent the cut-off call may not have finished (re-run it after
checking a spawn or commit did not already land), word it for any
provider, and keep a user's own stop and a quoted bb system tag out of
it. Test the note in each prompt builder.
