auto-review: carry post-review work instead of dropping it

Review fixes for the one-review-per-user-message rule:

- A queued review that is cancelled no longer counts as sent; it clears
  lastCodeReviewSeq like a failed send does.
- The already-reviewed stand-down keeps the turn cursor open (keepCursor,
  shared with carryOver), so work from wake-ups and tasks other threads tell
  is reviewed and committed by the review the next user message earns,
  instead of falling out of every later review.
- The user-turn lookup pages the event log oldest-first and fails open at its
  cap, so a long run of wake-ups cannot hide a user message.
- EVENT_PAGE_MAX now lives in detect-timeline.ts; plan.ts re-exports it.
- Docs state exactly what is carried; tests cover cancel, carry-over of edits
  and commits, deferred unparking, user turns during a review, and the cap.
