fix: only rebalance to a region that has nodes (#2290)

* fix: only rebalance to a region that has nodes

Rebalancer.check/3 reported wrong_region whenever the tenant's expected
region differed from this node's, even when no node belongs to it and a
new connection would start on the default node again. Connect and the
Postgres Changes tree then restarted on every rebalance check, which is
what happens on self-hosted and CLI nodes since REGION defaults to
"local".

Closes #2289

* chore: Use Realtime.Nodes.region() as suggested by its docs, which also will never be `nil`

* chore: Make the decision not to rebalance when there are no nodes a little clearer
