* feat: page opted-in Monitor handsets on logged errors

When an error or a PHP fatal is logged, alert the Monitor handsets an
admin has opted in on Monitor Devices. Step 2 of the Hand-to-Monitor
migration.

Each error is stored as one alert per opted-in handset, addressed to that
handset, so neither push nor poll reaches anyone else. They share one
message id and are informational. Title "[channel] LEVEL", body the
message plus file:line; the log context is never sent.

A cooldown per distinct error (digits and hex folded) and an hourly cap
hold repeats back, and the next alert reports how many. The wp and
monitor channels are excluded by default. Delivery happens at shutdown,
after the response, and needs nothing from Unity.

Also fixes the logger never recording fatals while WordPress's fatal
error handler is on: that handler's wp_die() exits and cancels the
logger's shutdown handler, so the fatal is now captured from the
wp_die() wrapper first, once only.

* fix: deliver error alerts on the shutdown action

On the SiteGround test bed something exits from an earlier shutdown function, which cancels every shutdown function after it, so a delivery registered at the end of the queue never ran: a logged error raised no alert there while a fatal (sent at once) did. Deliver on WordPress's shutdown action instead, at the latest priority, with a plain shutdown function as the fallback.
