feat: host the Monitor app's server side (#61)

Move the Hand app's server side out of Reach into an optional Monitor
module, as the first step of renaming the app to Monitor and paging
opted-in handsets when errors are logged.

The module lives under src/Monitor and serves sentinel/v1: device
sign-in (Google, Microsoft or Facebook through the browser leg, or
password), sealed FCM push, alerts, acknowledgement, contact, compose,
reply, resend and the member directory, plus the Devices, Message and
Settings admin pages under the Sentinel menu.

Authentication is unchanged. The SSO callback still applies Reach's
outreach rule before Monitor's own gate, and refuses anonymised relay
addresses first. The names and key domains are new, so nothing carries
over from Reach: handsets re-enrol and credentials are entered afresh.

It boots on unity/loaded and stands down without Scrutiny or Guardian,
or when SENTINEL_MONITOR_KILL is set, so the logger and dashboard keep
needing nothing. One behaviour differs from Reach on purpose: a member
lookup that throws answers 503 and leaves the handset enrolled.

Guardian ^1.0 is bundled, and CI checks Unity and Scrutiny out beside
Sentinel for PHPStan and the tests.
