Accept optional applications in application/0 (#169)

Mix documents `{app, :optional}` for `:extra_applications`, and also
accepts it in `:applications`. `fetch/0` wrapped every entry as
`{entry, []}`, so the tuple became `{{:tools, :optional}, []}` and
`normalize_dep/1` raised a FunctionClauseError.

Normalize both keys to `{app, opts}` before deduplication. This records
the optionality through the existing `optional:` option. It also keeps
one component per app, because `Enum.uniq_by/2` keys on the app name
instead of the raw tuple.

`:included_applications` keeps its bare-atom mapping, because Mix
rejects the tuple form for that key.
