Take a server URL's prefix off route paths (#7)

## Overview

Adds a prefix option to both adapters, for projects whose attribute
paths are written relative to a server URL that carries it, for example
`/users` under `https://example.com/api`. Stacked on #6, so this targets
`fix/route-collisions` until #5 and #6 are merged.

## Summary

- Routes added to the document now sit at paths relative to the server,
matching the documented ones, instead of resolving to the prefix twice
- An attribute declared away from its handler now finds its route by its
server-relative path, so it is reported as matched rather than unrouted,
with the route added again beside it
- Only whole segments come off, and routes outside the prefix keep their
paths
- The inventory reports the same paths the document has
- The README explains the option next to route filtering, and points to
it from the conflict status
