Carry over stored values for uncrossed NSGA2 children

An uncrossed child is a copy of an already-evaluated row, but crossover
writes it into a different slot, whose stored objectives/constraints
belong to the previous row. The last commit flagged such copies for
re-evaluation, which re-runs the model on known designs: about
(1 - Pc) * N extra evaluations per generation.

Instead, have simulated_binary_crossover return the source row of each
output slot and reorder objs_proposal/constrs_proposal by it, as
tournament selection already does. Copies keep their own stored values
and only crossed or mutated children are re-evaluated.

Add a test with Pc = Pm = 0 that checks only the initial population and
the final design are evaluated (21 runs; the previous code made 96) and
that Pareto objectives still match their designs.

Also clarify the constrained test problem's docstring (g_two sets the
feasible region) and note OpenMDAO >= 3.45.1 where docstrings describe
None for unbounded sides; 3.45.0 still uses the 1e30 sentinel.
