Revise setuptools-scm fallback version (#3487)

### Summary

Bump the `fallback_version` in `setuptools-scm` configuration, which was
not updated in the recent releases. Usually this is not an issue, but in
environments where `setuptools-scm` does not have access to all the
repository information (such as shallow clones, or missing the tags), it
would result in the package version incorrectly starting from the value
set.

Additionally, updates the release documentation to increase the chances
of ensuring it is tackled in next releases.
