perf(secure-channel): SecureChunkWriter for every secure channel message

MessageChunker writes every message (MSG, CLO, OpenSecureChannel; None,
Sign, SignAndEncrypt; local or remote HSM/KMS signature) with a
SecureChunkWriter: one call per message, no event, no chunk manager per
message, the same bytes on the wire (ADR 0001). Symmetric writers are
kept per token; ChunkManager, SecureMessageChunkManager and prepareChunk
stay exported, deprecated.

Messages chunked while an OPN waits for its remote signature are queued
behind it, in sequence order, and every chunk is sealed before the first
is sent. Tests compare the bytes with the chunk managers for every
policy, mode and key size.
