Restore shared template context on the MCP documentation page

Pass the request App to the MCP template so the shared rendering hook supplies fingerprinted asset URLs and the reference menu. Add route regressions for the rendered page and served stylesheet, and confirm JSON-RPC POST and SSE rejection still behave correctly.
