client: restart a process after an indeterminate operation

An operation that crashed an invoke is indeterminate: it may or
may not have taken place. Record it as an `info` completion
(instead of `fail`) with an `indeterminate` error, as in Jepsen,
and give the thread a new process identifier.

A client that is not reusable is reopened with close and open
before the next operation; a failed reconnect is recorded as a
`fail` operation and retried. Clients are not reusable by default,
but may implement a `reusable` method. The process counter lives
in a shared run context.

Failed client stages (open, setup, teardown, close) keep raising
an error and fail-stopping the pool, as before.
