Speed up C++ live tests (#846)

The C++ live tests took about 400 CPU-seconds per case, mostly for
work which the tests do not need.

Every generated test defined ``CATCH_CONFIG_MAIN``, so each test
translation unit compiled the whole Catch2 implementation (about 18
seconds each). We now generate a single ``test/catch_main.cpp`` and
link all the tests against it as ``add_library(catch_main OBJECT
test/catch_main.cpp)``.

The live test no longer builds the shared library and the C++11
compatibility check by default (``EXCLUDE_FROM_ALL ON``). The tests
link only against the static library, and the fast check already
compiles the C++11 target with ``-fsyntax-only``.

Finally, we compile with ccache if it is available, and set
``CCACHE_BASEDIR`` and ``CCACHE_NOHASHDIR`` so that the cache is hit
across different output directories. Re-building an unchanged case
takes 8 seconds instead of over 3 minutes.
