Count triggers per account only, and drop plan.rate_limit_key (#2679)

* Count triggers per account only, and deprecate FLEXMEASURES_API_RATE_LIMIT_KEY

Counting triggers per account and asset, or per user, made plans harder to show and to support.
The setting is still accepted, but ignored, with a warning at startup if it asks for anything other than "account".

* Drop plan.rate_limit_key, now that triggers are always counted per account

Removes the column and its enum type in a migration, the --rate-limit-key option from add plan and edit plan, the column from show plans, and the trigger scope from the account page.

Change: Display the plan name and effective API and trigger rate limits after account roles.

Context: Show account now includes the plan and effective limits.

Change: Extend the existing CLI test to verify server defaults and section order.

Context: PR 2679's CLI changelog entry already covers plan rate limits.

Change: Extend that entry with show account output and server defaults.

Context: The account plan output is easier to scan with a separator.
