Fold _md_fresh_kind and _md_next_kind into their callers (#1771)

## What do these changes do?

`_md_fresh_kind()` always returned `KIND_COMPACT` and ignored its
argument, and `_md_next_kind()` was a one-line ternary on top of it.
Both are folded into their callers in `hashtable.h`; the comment
explaining how a table picks its kind moves to `_md_rebuild()`. C
extension only; the pure-Python backend has no table kinds.

## Are there changes in behavior for the user?

No.

## Is it a substantial burden for the maintainers to support this?

No, it removes two helpers.
