feat: turn off per-file Size/Modified metadata by default (#32)

Breaking default for v0.11.0. Opt back in with --file-metadata or
file_metadata = true (explicit CLI wins). The content hash and the
auto-diff cache stay based on file bytes, so an mtime-only change no
longer rewrites the document or shows up as an edit.
