feat: remove the >100-file confirmation prompt (v0.11.0) (#34)

* fix: skip confirm prompts when stdin is not a terminal

Non-interactive runs with more than 100 files aborted on EOF
(context-builder </dev/null) or hung on an open pipe
(sleep 10 | context-builder). The overwrite prompt did the same.
Both prompts now proceed when stdin is not a TTY, matching -o -,
and interactive prompts are written to stderr.

* feat: remove the >100-file confirmation prompt

File count is a poor proxy for cost. Large outputs keep the existing
non-blocking warning when the estimate exceeds 128K tokens, and that
warning now goes entirely to stderr. --yes still skips the TTY
overwrite prompt and stays accepted for existing scripts.
