fix: honor comma-separated --ignore and fix the 128K advice (B12) (#26)

* fix: split --ignore on commas and correct the 128K advice (B12)

`--ignore a,b` was one pattern, so the tool's own advice to run
`--ignore docs,assets` ignored nothing. Comma-separated values now match
`--filter`, repeated `-i` still appends, and help/README document
gitignore globs and paths. The overflow warning suggests extensions from
the run instead of a hardcoded Rust filter.
