fix: skip confirm prompts when stdin is not a terminal (B4) (#25)

* fix: skip confirm prompts when stdin is not a terminal

Non-interactive runs with more than 100 files aborted on EOF
(context-builder </dev/null) or hung on an open pipe
(sleep 10 | context-builder). The overwrite prompt did the same.
Both prompts now proceed when stdin is not a TTY, matching -o -,
and interactive prompts are written to stderr.
