refactor(routing): seal controller middleware

- Rework Controller into an embeddable struct backed by sealed
  ControllerMiddleware storage (MiddlewareString / Middleware), keeping the
  Laravel-aligned Middleware/Only/Except registration semantics
- Drop the redundant middlewareString alias in favor of MiddlewareString
- Add the http_helpers.go implementation required by the already-tracked
  http_helpers_test.go (trusted proxy handling)
- Normalize documentation comments across the package
