ci(release): switch to version bump PR and publish-on-merge flow (#9)

Adopt the release flow from PeculiarVentures/pvtsutils:

- Version Bump workflow opens a PR that bumps the version and prepends
  CHANGELOG.md via conventional-changelog (angular preset)
- Publish workflow runs on package.json changes to main, publishes to npm
  via Trusted Publisher, pushes the version and major tags, and creates a
  GitHub Release from the matching CHANGELOG.md section
- Generate CHANGELOG.md for existing releases and exclude it from oxfmt
- Set publishConfig.access to public
- Document the new flow in CONTRIBUTING.md and AGENTS.md
