Add web-e-stepper: program pane + environment diagram (#131)

* Add web-e-stepper: program pane + environment diagram

Self-contained host plugin for the environment stepper (#121): step slider
and explanation, the program (reusing the stepper's renderer, now in its
own module with a hook for host-rendered node types) with EnvBlock brackets
and Ref badges, a collapsible output strip, and a Konva environment diagram
(frames by depth, function objects and lists, arrows, active frame, lookups,
garbage greyed). Narrow tab stacks the panes; wide tab is side by side.

ResizeObserver's first callback can be delayed (e.g. in a background tab),
which left the diagram unrendered until the tab was resized.

* Tests for the stepper renderer, the environment diagram and the view

render.tsx (moved out of SubstVisualizer.tsx, which no test loaded) is now
counted by coverage; test its built-in and profile-driven rendering. Draw
the environment diagram with Konva stubs that render their children, and
mount it from the view through a node mock.
