Allow display attributes to inherit colors and settings (#1409)

A foreground or background of `'inherit'` takes the color from the
enclosing attribute, so a container sets a background its children keep
without a palette entry per fg x bg combination.

Fixes #329

- AttrSpec and palette entries accept `'inherit'`; `'no-bold'` and the like
  turn a setting off over the enclosing attribute.
- Nested markup and AttrMap record the enclosing attributes as a
  LayeredAttr; AttrMap can also map a whole LayeredAttr.
- BaseScreen.resolve_attr() resolves any run attribute; all bundled
  screens use it, and the raw and web screens redraw only the rows a
  palette change gives other colors.
- ANSIText leaves colors it does not set to the enclosing attribute and
  keeps SGR resets as 'no-' settings; SGR 3/23 and 9/29 (italics,
  strikethrough) are parsed for ANSIText and Terminal.
- AttrSpec.copy_modified() can add a color to a spec without one.
- examples/input_test.py uses a dark Monokai palette with inherited
  backgrounds.

Breaking: canvas content and Text.get_text() carry LayeredAttr for
nested attributes.
Custom screens have to call resolve_attr() instead of reading
the palette.
An attribute an AttrMap maps to None shows the map's None attribute.
