Make the legacy documentation migration notice prominent (#1407)

Replace the grey in-page note with a prominent migration banner above the
Sphinx page layout. Include the banner on search, index, and generated API
pages, and preserve the existing notice for non-HTML documentation.

Refs #1405. The 2.3 backport is submitted separately against 2.3_branch.

Validation: HTML and LaTeX builds passed; all 13 HTML pages contain the banner.
Desktop and mobile rendering was checked. All 487 documentation doctests,
formatting, lint, and type checks passed.
